About the Role:

The Referral Coordinator plays a critical role in facilitating seamless patient care by managing and coordinating referrals within the clinic. This position ensures that patients are connected to the appropriate specialists, services, or resources in a timely and efficient manner. The Referral Coordinator acts as a liaison between healthcare providers, patients, and external organizations to streamline communication and follow-up processes. By maintaining accurate records and tracking referral statuses, the coordinator helps improve patient outcomes and satisfaction. Ultimately, this role supports the overall healthcare delivery process by reducing delays and enhancing care continuity.

Minimum Qualifications:

  • High school diploma or equivalent required; Associate’s degree or higher in healthcare administration or related field preferred.
  • Minimum of 1-2 years experience in a healthcare setting, preferably in patient coordination or referral management.
  • Familiarity with medical terminology and healthcare referral processes.
  • Proficiency in using electronic health records (EHR) systems and standard office software.
  • Strong organizational and communication skills.

Preferred Qualifications:

  • Bachelor’s degree in healthcare administration, social work, or a related discipline.
  • Experience working with insurance verification and authorization procedures.
  • Knowledge of healthcare regulations such as HIPAA and patient privacy standards.
  • Previous experience in a multidisciplinary healthcare environment.
  • Certification in medical office administration or patient coordination.

Responsibilities:

  • Manage incoming referral requests from referral partners and verify patient eligibility
  • Coordinate with specialists, clinics, and external service providers to schedule appointments and ensure timely patient access to care.
  • Maintain detailed and accurate records of all referrals, including tracking status updates and outcomes.
  • Communicate effectively with patients to provide referral information, answer questions, and assist with any barriers to care.
  • Collaborate with internal teams and external partners to resolve referral issues and improve referral processes.
  • Monitor referral workflows and generate reports to identify trends and areas for process improvement.
  • Ensure compliance with healthcare regulations, privacy laws, and organizational policies related to patient referrals.
