Registered Nurse (RN)

Requirements:
- Active Registered Nurse License in the state of Ohio
- Tuberculosis (TB) screening/QuantiFERON/Chest X-Ray (within 1 year)
- Proof of U.S. work authorization (valid, unexpired documents: government issued ID)
- Reliable transportation
Responsibilities:
- Conducting Start of Care (SOC) visits to evaluate medical, physical, and emotional needs and creating individualized care plans.
- Performing regular reassessments to monitor progress and adjust care plans as needed.
- Providing patient and family education on condition management, medication adherence, and preventive health measures.
- Collaborating with physicians, therapists, social workers, and families to ensure coordinated, comprehensive care.
- Maintaining accurate clinical documentation in compliance with Ohio Board of Nursing regulations and agency policies.
- Supervising and supporting Certified Home Health Aides (CHHAs) and other care team members.

SKILLED NURSING RATES– MEDICAL - includes visit, follow up call, all documentation
SOC OASIS: $150 (No more than 2 per day)
ROC OASIS: $125 - (Resumption Of Care after inpatient hospitalization of more than 24 hours)
RECERT OASIS: $125 – Required every 60 days for re-certification of care plan, reassess patient medical needs, physical assessment, review med changes.
DISCHARGE OASIS: $75 - Includes discharge summary (then is sent to MD)
TRANSFER OASIS: $75 - Includes summary of care provided and update of patient (then is sent to MD AND INPATIENT FACILITY
REVISTS: $60
SUPERVISIONS: INCLUDED IN REVISIT RATE (LPN SUPERVISIONS NO LESS THAN EVERY 30 DAYS, HOME HEALTH AIDE SUPS NO LESS THAN EVERY 14 DAYS)
ON CALL VISITS: $100
NON-BILLABLE RN: $45
NON-BILLABLE LPN: $25
NON MEDICAL NURSING RATES:
SOC ADMISSION: $125 (includes full documentation in HHAX including creating 485)
60 DAY REASSESS: $75 (includes full documentation in HHAX including creating 485) AND SUPERVISIONS
REHOSP VISIT: $75 (includes care plan review and changes in POC when necessary)
NON-BILLABLE RN: $45
NON-BILLABLE LPN: $25
