Job Title: Nurse Practitioner (Full-Time)- Montgomery County, MD
Full Time/Days
Pay- $160K Annually based on exp

Job Summary
We are seeking a dedicated and compassionate Nurse Practitioner to provide high-quality, home-based primary care services to geriatric patients. This role involves delivering comprehensive care to homebound individuals, supporting their medical needs, and ensuring continuity of care through collaboration with interdisciplinary teams. The ideal candidate is experienced in geriatric care or motivated to develop expertise in this area.
Key Responsibilities

Provide primary care services to patients in a home-based setting

Conduct comprehensive physical assessments, diagnoses, and treatment planning

Deliver direct and indirect patient care to manage acute and chronic conditions

Prescribe medications and implement appropriate clinical interventions

Develop, monitor, and adjust individualized care plans

Review and interpret diagnostic tests, consult notes, and discharge summaries

Write medical orders and manage referrals to specialists and services
-
Educate patients and families on health conditions, treatment plans, and preventive care

Collaborate with interdisciplinary healthcare professionals to coordinate care

Maintain accurate and timely documentation in electronic health records
Qualifications

Current licensure as a Nurse Practitioner

Master's or Doctorate degree in Nursing from an accredited program
-
Certification as an Adult, Family, or Gerontology Nurse Practitioner (e.g., ANCC, AANP)

Active prescriptive authority and DEA registration
-
Valid driver's license and current CPR certification

Minimum of 2 years of Nurse Practitioner experience

Strong clinical judgment and independent decision-making skills

Excellent communication and interpersonal skills
Preferred Qualifications

Experience in home-based care or working with geriatric populations

Familiarity with Medicare patients and care requirements

Experience using Electronic Health Record (EHR) systems
