Job Title: RN Nurse Care Manager – Hybrid in Westchester
Location: Westchester, NY
Work Model: Hybrid – field visits in Westchester territory+ remote work
Onsite Training in Bronx, NY for 2-4weeks
Job Type: FTE/Permanent

About the Role
We are seeking a Registered Nurse (RN) Care Manager for a hybrid position supporting older adults and individuals with complex healthcare needs. This role combines limited field work with remote care coordination, offering strong work-life balance while making a meaningful impact in the community.

What You’ll Do
Conduct in-home assessments to evaluate members’ clinical, functional, and social needs
Develop and manage individualized care plans in collaboration with physicians and interdisciplinary teams
Coordinate care across home care, hospital, rehabilitation, and long-term care settings
Complete approximately 6 home visits per week within Brooklyn
Monitor patient progress and adjust care plans to ensure optimal outcomes
Support safe discharge planning and reduce hospital readmissions
Communicate with members, families, caregivers, and providers
Maintain accurate and timely documentation in compliance with guidelines

Work Schedule & Structure
Hybrid role: Field + Remote
Field work: ~6 home visits completed in 1 day (West Chester territory)
Remote work: Care coordination, documentation, and follow-ups
Training location: Bronx office
Orientation: Bronx

Qualifications
Active, unrestricted RN license in New York
Associate’s degree in nursing required (BSN preferred)
Minimum 3 years of RN experience
Experience in MLTC, home care, geriatrics, or case management preferred
Strong communication, organizational, and critical thinking skills
Ability to work independently in a hybrid environment
Bilingual skills are a plus
