Overview
We are seeking a dedicated and reliable LPN or RN to join our Home Health team. The ideal candidate will provide high-quality nursing care to our peds. patients in their homes, ensuring their comfort and well-being. This role requires a strong clinical background, excellent communication skills, and the ability to work independently while collaborating with a multidisciplinary team. A strong knowledge of trach care, G-tube care, is a plus. RELIABLITY IS A MUST
Responsibilities
- Conduct comprehensive patient assessments (RN Only) to develop individualized care plans.
- Administer medications and treatments as prescribed.
- Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
- Provide education and support to patients and their families regarding health management and treatment options.
- Document all patient interactions, assessments, and care provided in compliance with regulatory standards.
- Collaborate with physicians, and other healthcare professionals to coordinate patient care.
- Maintain a safe and clean environment for patients, adhering to infection control protocols.
Experience
- Previous experience in home health working with medical fragil children., G-tube and Trach knowledge is a plus, will train.
- Excellent interpersonal skills with the ability to build rapport with patients and families.
- Ability to work independently while effectively managing time and priorities in a home health setting.
