Hospice RN Admissions Nurse – Weekend/Night On-Call (PRN)

Are you a compassionate, skilled Registered Nurse with a passion for providing exceptional end-of-life care? Advanced Home Health & Hospice of Arizona is seeking an experienced Hospice RN Admissions Nurse to join our dedicated team on a weekend, night, on-call PRN basis.

Position Details

• Schedule: Weekend/Nights, On-Call (PRN)
• Compensation: Paid per visit
• Location: Various service areas throughout Arizona

Responsibilities

• Conduct hospice admissions, ensuring patients and families understand the hospice philosophy and services.
• Complete thorough nursing assessments and develop individualized plans of care.
• Collaborate closely with the interdisciplinary team to coordinate timely, compassionate care.
• Provide emotional support and education to patients and families.
• Accurately document patient assessments and care provided in accordance with regulatory and agency standards.

Requirements:

• Current Arizona RN License (required)
• Minimum 1 year of hospice nursing experience (required)
• Strong clinical assessment and communication skills
• Ability to work independently during night and weekend hours
• Compassionate, patient-centered approach to care
