Care Central Professionals Home Health is seeking a compassionate and skilled Per Diem Physical Therapy Assistant (PTA) to join our team in Roseville, CA. This role offers flexibility and the opportunity to provide one-on-one care that helps patients regain independence and improve their quality of life in the comfort of their homes.

Responsibilities:

• Provide physical therapy services under the supervision and direction of a licensed Physical Therapist (PT).

• Carry out individualized treatment plans to improve mobility, strength, balance, and functional independence.

• Educate patients and caregivers on safe exercise techniques, mobility strategies, and home safety.

• Accurately document visits and patient progress in compliance with agency and regulatory standards.

• Collaborate with the care team to ensure patient-centered, coordinated care.

Qualifications:

• Current California PTA license in good standing.

• Graduate of an accredited Physical Therapist Assistant program.

• Minimum of 1 year of clinical experience (home health experience preferred).

• Strong communication and interpersonal skills.

• Valid driver’s license and reliable transportation.
