Shift: Day Shift | 10:00 AM – 6:00 PM
Schedule: Monday – Friday
Hours: 40 hours per week
Pay Rate: $55 – $61/hour
Estimated Weekly Pay: $2,062.50 – $2,287.50
Position Overview
"Navitas Healthcare, LLC" is seeking an experienced and detail-oriented Registered Nurse (RN) Care Manager to support care coordination within a fast-paced Emergency Department setting in the Bronx. This role focuses on utilization review, discharge planning, and patient advocacy to ensure safe, timely, and appropriate transitions of care. The ideal candidate demonstrates strong clinical judgment, organizational skills, and interdisciplinary collaboration.
Key Responsibilities

Collaborate with Emergency Department clinical teams to coordinate patient care and transitions

Perform utilization review to assess medical necessity and appropriate level of care

Develop and implement effective discharge plans, including home care, sub-acute placement, or outpatient follow-up

Advocate for patients by coordinating with families, providers, and payers
-
Ensure accurate and timely documentation in electronic medical records (EMR)

Support compliance with regulatory and hospital care management standards
Qualifications
Required
-
Minimum 5 years of clinical experience as a Registered Nurse

Demonstrated experience in Care Coordination, Utilization Review, and Discharge Planning

Associate Degree in Nursing (ASN/ADN) from an accredited institution
-
Active New York State RN License with primary source verification (NYSED)

BLS Certification
-
PRI and Screen Certification (required upon hire or within 3 months)

Strong computer skills and familiarity with EMR systems
Preferred

BSN or MSN

Prior leadership or supervisory experience

Certified Case Manager (CCM) or equivalent certification
