The Hospice Clinical Team Lead is the primary on-site clinical leader for an assigned hospice location. This role combines direct patient care with clinical leadership, staff supervision, and operational coordination. The Clinical Team Lead provides oversight and support to the Registered Nurse Case Manager and Home Health Aides, serves as the liaison between the facility and hospice leadership, and ensures consistent, high-quality, and compliant hospice care.

Schedule: Monday– Friday 8:30 am–4:30 pm


Essential Job Functions & Responsibilities

Clinical Leadership & Supervision

  • Serve as the primary on-site clinical leader responsible for the coordination and management of hospice services at the assigned location.
  • Provide day-to-day supervision, leadership, and support to the RN Case Manager and Home Health Aides.
  • Conduct supervisory visits, observe clinical performance, and provide coaching and feedback.

Direct Patient Care

  • Maintain an active hospice patient caseload while fulfilling leadership responsibilities.
  • Provide high-quality, compassionate hospice nursing care in accordance with professional standards and organizational policies.
  • Serve as a clinical role model through professional practice, communication, and patient-centered care.
  • Adjust personal caseload as needed to ensure continuity of care and operational stability.

Quality, Compliance & Documentation Oversight

  • Ensure care and services are delivered appropriately and in compliance with hospice regulations, professional standards, and organizational policies.
  • Review clinical documentation for accuracy, completeness, and timeliness, including care plans, medication records, and discharge documentation.
  • Collect and review pertinent clinical data and participate in performance improvement initiatives.
  • Assist with the development, implementation, and monitoring of clinical policies and procedures, including recommending updates as needed.
  • Participate in and provide leadership during regular Interdisciplinary Team meetings.

Facility Liaison & Service Recovery

  • Serve as the primary liaison between the facility and Catholic Community Hospice leadership.
  • Build and maintain strong, professional relationships with facility leadership and staff.
  • Address patient, family, and facility concerns or complaints promptly, professionally, and compassionately.
  • Escalate concerns appropriately and partner with leadership to ensure effective service recovery and resolution.

Requirements:

  • Valid RN license required.
  • Hospice experience preferred.
