About the Role

Join our dynamic healthcare group as a Post-Acute Care Medical Assistant and serve as a primary liaison for patients in our innovative care model that focuses on safe transitions from hospital to assignments. Reporting to the Director of Post-Acute Care, you will be a member of a respected and highly functioning team with support to help you grow in your role.


This is a full-time (40h/week), hourly (non-exempt) hybrid position.


To support the growing needs of our patients and our Transitions of Care team, this role may occasionally require patient home visits (in surrounding areas). Personal vehicle use required; reimbursement provided.


Applicants must currently live in the Fort Collins, Timnath, Windsor, Loveland or Severance area.

Requirements:

Responsibilities

  • Support physicians and APPs with their daily workflows including scheduling appointments, medication reconciliations, triaging clinical questions from facilities, medication refills, and reporting vital signs
  • Answers phones on a scheduled rotation with the other office staff
  • Coordinates and maintains paperwork workflows such as faxes, letters for patients, and orders requiring signatures from providers
  • Support our patients and the Transitions of Care team by visiting patients in their homes to take vitals, draw labs and assist the physician during home visits
  • Identify patients that would benefit from transition of care phone calls and coordinate provider visits
  • Coordinate patient care needs and schedules with the SNFs, ALFs, and IRFs
  • Collaborate with healthcare providers and other staff members to clarify documentation and resolve discrepancies
  • Adhere to confidentiality and privacy policies to protect sensitive patient information
  • Provides exceptional and prompt customer service to patients, families, facility staff, and third-party vendors
  • Additional duties as assigned

What We’re Looking For

Qualifications

  • Training as Licensed Practical Nurse (LPN), Medical Assistant (MA), or Registered Nurse (RN) required
  • Must reside in the Fort Collins, Timnath, Windsor, Severance or Loveland Area
  • Reliable transportation
  • Proficiency in electronic health record (EHR) systems (EPIC preferred)
  • Strong knowledge of medical terminology, anatomy, physiology, and disease processes
  • High degree of accuracy, attention to detail and analytical skills
  • Agile, with an ability to prioritize and flex through the day-to-day demands with composure and professionalism
  • Demonstrated ability to work independently and collaboratively
  • Strong written and verbal communication and interpersonal skills
  • Passion for building and cultivating positive relationships with patients, colleagues, and external vendors
  • Continuous improvement mindset, with a drive for enhancing and delivering exceptional customer service to our patients
  • Customer-centric approach that contributes to a welcoming and collaborative atmosphere within the group
  • Proficiency in Microsoft Office products (Word, Excel, Outlook, Teams, etc.)
  • Bilingual candidates who are proficient in Spanish are highly preferred!

Physical/Mental Demands & Work Environment

  • Requires sitting and standing associated with a normal office environment
  • Performs highly complex and varied tasks requiring independent knowledge and its application to a variety of situations, as well as exercising independent judgement
  • Requires the use of office equipment such as a computer and telephone
  • May view computer screens for prolonged periods of time
  • Must be able to prioritize activities when faced with competing demands
  • This position will have remote work capabilities and must have the skill set to work independently, exercising sound judgement while maintaining high productivity levels
  • Requires elevated levels of electronic and telephone communication
