The Community Health RN Supervisor will be responsible for the oversight and coordination of the Community Health services ensuring that a viable network of health and social service resources are available to compliment the Wellness Center’s goal of delivering high quality care to eligible tribal members. The Community Health scope of practice includes, but is not limited to prevention, health promotion, case identification, education, service coordination and advocacy for individuals and families in the local community. Routine duties shall include providing health care related services to individuals eligible for services provided by Klamath Tribal Health & Family Services in homes, schools, clinics, alternate job sites and other community locations within the Klamath Tribes’ Service Unit.

Requirements:

MAJOR DUTIES AND RESPONSIBILITIES

1. Responsible for program development and implementation of Community Health services. Community Health programs include comprehensive community health nursing activities related to such services as diabetes prevention and care, accident prevention, nutrition, communicable disease control, maternal and child health care, breast cancer awareness, acute and chronic diseases, physical and emotional fitness and other programs as assigned.

2. Responsible for the management of the Community Health program. This includes developing annual operating plans; analyzing expected workload; determining how program services should be provided and working with the Lead Nurse to determine budget needs.

3. Responsible for establishing internal mechanisms that adhere to the required credential and accreditation processes for department staff, including but not limited to practice guidelines, ethical guidelines, quality assurance, appropriate utilization of services, peer review, and regularly scheduled case conference/management meetings.

4. Based upon Tribal vision and priorities and management direction develop comprehensive health programs by coordinating and participating in the promotion, planning and implementation of community health programs. Work with other disciplines and agencies in keeping with local, state, and national standards.

5. Develop treatment protocols and establish policies and procedures for Community Health services in consultation with management and medical staff.

6. Supervise the Community Health Representatives in pursuit of attaining the Community Health program goals and responsibilities. Exercise full range of supervisory duties and program activities, which includes providing clear direction for all Community Health Representatives and evaluating job performance.

7. Develop community health plans by utilizing the case management approach incorporating the nursing process model. Carry out nursing plans through home visits, office visits, school visits, outreach clinics and group education.

8. Participate in Community Health caseload clinical staffing on a weekly basis with the Nursing Supervisor. Coordinate and participate in patient care conferences with medical, educational, and social service resources for the purpose of providing integrated care to tribal members.

9. Work cooperatively within own department and with other tribal programs to create a system of quality health care delivery.

10. Assist all departments in diabetes education and services to patients through workshops and/or individual counseling sessions. Develop and implement educational lessons based on individual learning needs.

11. Aid individuals and families to accept and adjust positively to physical, mental, and social limitations. Assist the family to accept and assume the responsibility for providing and/or arranging for care and guidance toward self-help.

12. Maintain records and reports on patients, patient care plans, monthly service report, and other required information for use in program operation, quality assurance and other purposes.

13. Provide backup coverage for the medical clinic as directed by supervisor in times of short staffing.

14. Like employees of the Klamath Tribes, the incumbent will be called upon to accomplish other tasks that may not be directly related to this position, but are integral to the Klamath Tribes’ broader functions, including but not limited to, assisting during Tribal sponsored cultural, traditional, or community events that enable the successful operation of programs and practices of The Klamath Tribes as aligned with The Klamath Tribes’ Mission Statement. Some of these tasks may be scheduled outside of regular work hours, if necessary.
