Home Care Network (HCN) is urgently seeking a highly skilled and experienced Home Health RN Case Manager to join our agency to deliver and lead exceptional care for our existing and new clients across Southeast Ohio, primarily serving Jackson, Vinton, and Gallia Counties.

This field staff leadership role includes starts of care (SOC), recertifications, resumptions of care (ROC), and other assessments and services, as well as new hire orientations, employee reviews, and input to influence our training and education, care practices and outcomes, and the success of our clients, employees and business overall. The right RNCM has a passion for delivering consistently excellent care and care experiences to clients, and the ability to do so directly and through the thoughtful leadership of others.


More about the role:

  • This is a Full-Time, Salaried position.
  • Expertise in wound care, case management, IV, lab draws, OASIS, and HHA supervision i required (1+ years minimum of each).
  • Minimum availability required: Monday - Friday from 8 AM - 5 PM.
    • No On-Call requirements / No Overnight shifts required.
  • The typical travel radius is 30-60 minutes, with occasional travel beyond for relief/backup (no overnight travel).
  • Door-to-Door mileage reimbursement provided.

Compensation: FT RNCM salary starts at $72,800 per year, with quarterly bonus opportunities available.


Qualifications:

  • Graduate of an accredited School of Nursing.
  • Minimum of 1 year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.
  • 1+ years of paid experience in case management is required, with a home health agency or equivalent environment highly preferred.
  • Additional experience requirements include: IV, lab draws, OASIS, and HHA supervision (1+ years minimum of each).
    • Supervisory / DON experience is a plus.
  • Strong background in nursing, patient care, and confidentiality.
  • Strong attendance and dependability track record with previous employers.
  • Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, and positive.
  • Familiarity with HIPAA regulations and medical terminology.
  • CPR and First Aide Certifications required (or can be obtained prior to hire date through our agency or accredited third parties).
  • Current Driver's License and reliable transportation.


Job Summary
As an RN Case Manager, you will be instrumental in coordinating and providing patient care and ensuring optimal outcomes.

  • With core skills in managed care, patient assessment, client care and treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care.
  • Your premium skills in critical care and infectious disease care will be valuable in handling complex cases, while your relevant skills in home and community care and supervising experience will enhance your ability to lead and support a multidisciplinary team.
  • You will ensure the successful communication with and between our physicians, our agency staff, service administrators, other caregivers and clinicians, and the client + his/her family.

Reports to: Regional Administrator, or Clinical Administrator/Director of Nursing


Responsibilities/Requirements of an RN Case Manager / Clinical Supervisor include:

  • RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.
  • RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • RN to Document/Communicate/Coordinate patient status/change in condition to care team.
  • RN to perform job duties while promoting personal safety and a safe environment for clients and coworkers.
  • Willing to travel surrounding counties.
