Reports To: Social Service Supervisor
Work Hours: 40 hours/week (Monday–Friday)
Travel: Less than 10%
Status: Full-time, Non-Exempt

Salary Range:

  • $41,000–$47,000 with Bachelor's degree

  • $36,000–$42,000 with High School diploma

Job Summary

At Esperanza Health Center, our Social Service Consultants (SSCs) are an essential part of the care team. SSCs engage directly with patients—in person or over the phone—to identify needs and connect them to vital services. These include assistance with insurance enrollment, transportation, food access, utility support, legal referrals, and more. Working closely with our medical, behavioral health, and community health departments, SSCs play a key role in reducing barriers and promoting holistic wellness for our patients.

Key Responsibilities

  • Conduct patient assessments related to social determinants of health and provide culturally appropriate resources.

  • Maintain accurate documentation in the Electronic Medical Record (EMR).

  • Provide limited case management for an assigned panel of patients living with HIV.

  • Offer crisis intervention and referrals for domestic violence or child abuse cases.

  • Perform HIV counseling and testing in accordance with established guidelines.

  • Assist uninsured patients in accessing and applying for health insurance.

  • Support patients through insurance renewal processes.

  • Facilitate referrals for Early Intervention and Autism Evaluation services.
