Job Details
Home Health Physical Therapist seeing patients in homes.
Assessment: Home health physical therapists assess patients' physical conditions, mobility, and functional limitations to create personalized treatment plans.
Rehabilitation: They provide a range of rehabilitation exercises, therapeutic techniques, and interventions to help patients regain strength, mobility, and independence.
Education: Home health physical therapists educate patients and their families on proper techniques, exercises, and strategies for managing their conditions and preventing further issues.
Monitoring and Progress Tracking: They regularly monitor patients' progress, adjust treatment plans as needed, and work collaboratively with other healthcare professionals to ensure the best possible outcomes for patients receiving home-based care.
