Pharmacist
POSITION SUMMARY
• The Pharmacist is responsible for the filling and dispensing of medication orders including the proper handling, usage, and storage of drugs in the pharmacy.
• Maintains standards set by the department and translates policies and procedures into the daily operation of the pharmacy.
• Responsible for acting as an information resource to patients, physicians, nurses, and other healthcare professionals regarding medication therapy and drug-related information.
OVERVIEW
• The evaluation is to assure individual performance, departmental goals and organizational goals are aligned. It is designed to support communication between the manager and the employee. Employee perception of their own performance is very important. To maximize the benefit of this process, both the manager and the employee participate in the evaluation process.

B. Education
• High School diploma or GED
• Bachelor od science degree in pharmacy from an accredited institution required.
• Advanced degree preferred or enrollment in advanced degree program.
C. Licensure
• Current State of Georgia pharmacist license required.
D. Experience
• Minimum of one (1) to three (3) year’s experience as a registered pharmacist in a hospital preferred.
• ASHP accredited residency preferred.

• Shall be familiar with unti dose, I.V. admixtures and clinical pharmacy practice.
• Should have memory for details; alertness to detect errors in drug orders; initiative and judgement involving mathematical and pharmaceutical problems; prudent buying of drugs; furnishing information to physicians, nurses, and other health care professionals; and have management skills to organize daily operational duties of the pharmacy department.
• Must be willing to accept calls at night and be available in emergency situations.
• Must be willing to work a flexible work schedule including 12-hour shifts, weekends, and holidays.

JOB SPECIFIC DUTIES AND PERFORMANCE STANDARDS
• Below are those tasks, duties, and responsibilities that comprise the means of accomplishing the position’s purpose and objectives. o Pharmacist Duties and Responsibilities
Performs all responsibilities/duties required by the pharmacy as defined by the scope of service to ensure that the unique nature of the client is addresses.
Responsible for the preparation and distribution of medication orders filled for the in-patient, outpatient, and ancillary areas.
Responsible for training, supervising, educating, and checking the work of supportive personnel and students; and for
Responsible for ensuring the correct labeling of medication containers.
Responsible for all counseling of employees with their prescriptions. Responsible for counseling patients upon request.
Responsible for interpretation of physician medication orders for accuracy and appropriateness, informing them of any errors, omissions, or discrepancies and, based on an evaluation of the patient's condition, suggests appropriate or alternate drug therapy to the physician.
Responsible for maintaining accurate and proper patient medication profiles, monitoring to insure the proper choice of drug, dosage form, strength, length of therapy and interaction potential of the prescribed drug.
Responsible for maintaining a working knowledge of, and conforming to legal requirements concerning the distribution of drugs, as well as good working knowledge of recent developments in the field of pharmacy (self continuing education by attending available conferences, meetings and seminars).
Responsible for the development, preparation, and compounding of special dosage forms of drugs not commercially available.
Assumes "charge pharmacist" responsibilities as required, for the coordination of weekend and evening shifts which include supervising the part-time pharmacists and supportive personnel, assigning work and delegating tasks and responsibilities, and for handling problems that arise during the absence or unavailability of the director of pharmacy services.
Responsible for performing monthly nursing station checks to ensure that medications are properly and safely stored, that excess and out-dated drugs are removed, and that information regarding drug usage and administration is communicated to the nursing staff. Responsible to function as pharmacy liaison to assigned nursing units for the purpose of handling routine, day-to-day issues.
Responsible for the daily coordination of the manufacturing and distribution of sterile products, including intravenous admixtures, total parenteral nutrition solutions and miscellaneous products.
Maintains and assures quality control standards of the department by conducting drug usage evaluations (DUE) and quality assurance (QA) programs within guidelines of JCAHO.
Responsible for the distribution, control and record keeping of all controlled substances, and ethyl alcohol.
Responsible for conducting in-service education lectures for medical staff, nurses, pharmacists, students, and other health care personnel regarding drugs, procedures, and policies.
Responsible for intradepartmental communication by staff meeting and in-service attendance. Also responsible for knowing content of departmental staff meeting minutes and memos. Maintains a working knowledge of the pharmacy policy and procedure manual.
Maintains and updates the hospital formulary system according to hospital policy and suggests alternative medications to the prescribe when non-formulary drugs are ordered.
Responsible for proper communication of problems and/or actions taken are made known to management.
Responsible for special projects pertaining to specific areas of interest and/or expertise; for conducting in-depth and highly specific research reviews of drug-related literature and ability to make presentations on rational drug therapy.
Responsible to maintain, inventory and handle all investigational drugs according to federal, state, hospital, and departmental policies.
Prepares and writes in-depth drug administration guidelines for the hospital. Participates in providing and environment conducive to learning for students from the professional schools utilizing the hospital for clinical experience.
Performs other related duties as required to promote and otherwise develop the pharmacy department. May also perform work of a higher or lesser degree when workload of the department so requires.
Responsible for some inventory control/purchasing activity as required to maintain departmental inventory at appropriate levels.
