Performs professional behavioural health case management in a psychiatric outpatient setting by overseeing and participating in assisting the developmentally disabled, mentally ill or emotionally disturbed to readjust and achieve a social, economic, emotional, and physical balance and personal independence compatible with generally accepted norms. Professional behavioural health case management services include but are not limited to assessments, treatment planning, direct service provision, person-centred services and management of client risk and safety. Reports to an administrative superior and is responsible for providing comprehensive patient care services that support the integrity. Provides consultative services to other state and private agencies, as well as referring clients/patients to appropriate social service agencies. This role works as a bridge between Peer/Certified Peer and Behavioural Health Case Manager III
Minimum qualifications
- Bachelor’s degree in behavioural or social science or related field.
- One year of experience in case management
- One year of experience in providing clinical treatment or determining eligibility for services or benefits as part of a treatment team
