Responsibilities
- Collect data on residents’ physical and psychosocial condition and communicate findings to the RN and the interdisciplinary team.
- Contribute to the development of an age-appropriate, culturally relevant plan of care reflecting nursing diagnoses and interventions to achieve resident outcomes; involve the resident/family and healthcare team.
- Recommend revisions to the plan of care after evaluating resident outcomes and indicate the need for referrals to address all aspects of care; collaborate with residents/families and team to ensure satisfaction and safety.
- Provide direct and indirect skilled nursing care under the supervision of a Registered Nurse; maintain an appropriate level of knowledge and clinical expertise through ongoing education and in-service activities.
- Maintain current knowledge and competency in nursing practice; participate in mentoring and orientation of staff; contribute to safe, cost-conscious care delivery.
- Demonstrate leadership in the professional practice setting and collaborate with physicians and ancillary staff to support quality outcomes; address occupational exposure risks in accordance with policy.
Requirements
- Active New York Licensed Practical Nurse (LPN) license.
- Current Basic Life Support (BLS) certification through American Heart Association or RQI.
- Minimum 1 year of LPN experience, with at least 1 year in a long-term care facility.
- Ability to work evenings and participate in a schedule that includes weekends and holidays as required.
- Proficiency in data collection, assessment, and communication within an interdisciplinary team; ability to care for adult and geriatric residents.
- Commitment to continuing education and professional development; ability to follow facility policies and procedures under RN supervision.
