Job DescriptionRN Care Manager

Location: York, Maine
Job Type: Contract (13 Weeks)
Pay Rate: $2,100 - $2,500 Weekly (Based on Eligibility and Experience)
Weekly Hours: 40 Hours

Shift & Schedule:
• Option 1: Monday–Friday, 8:30 AM – 4:30 PM
• Option 2: Four (4) 10-Hour Shifts, 7:30 AM – 5:00 PM

Position Overview

We are seeking an experienced RN Care Manager to coordinate utilization management, discharge planning, and care transition activities across an acute care hospital setting. The ideal candidate will serve as a liaison between patients, healthcare providers, payers, and community resources to ensure safe, effective, and timely transitions of care.

Required Qualifications

• Active Registered Nurse (RN) License.

• Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN).

• Minimum three (3) years of clinical nursing experience.

• Recent utilization review and discharge planning experience required.

• Strong knowledge of healthcare reimbursement methodologies and quality measures.

• Excellent communication, organizational, and critical-thinking skills.

• Ability to collaborate effectively with multidisciplinary healthcare teams.

• BLS/BCLS Certification required.

• CCM (Certified Case Manager) preferred.

• ACM (Accredited Case Manager) preferred.

• CCMA preferred.

Responsibilities

• Conduct comprehensive patient assessments and identify discharge planning needs.

• Coordinate care transitions and secure appropriate post-acute services and resources.

• Collaborate with interdisciplinary teams during rounds, care conferences, and care planning meetings.

• Facilitate utilization review activities and concurrent case management.

• Work with insurance providers to obtain authorizations and approvals.

• Coordinate transportation, durable medical equipment, home health services, and community resources.

• Monitor patient length of stay and support efficient resource utilization.

• Ensure compliance with hospital policies, payer requirements, and regulatory standards.

• Maintain accurate documentation within electronic healthcare systems.

• Educate patients and families regarding care plans and discharge processes.
