Location: New York, NY
Contract Length: 13 Weeks
Schedule: Monday–Friday | 9:00 AM – 5:00 PM
Hours: 35 Hours per Week

Job Summary

"Navitas Healthcare, LLC" is seeking a Licensed Master Social Worker (LMSW) for a contract assignment in New York, NY. The LMSW will support inpatient medical and surgical patients by conducting psychosocial assessments, providing supportive counseling, coordinating discharge planning, and facilitating community resource referrals.

Responsibilities

• Conduct psychosocial assessments for inpatient medical and surgical patients

• Provide supportive counseling and therapeutic interventions

• Develop and coordinate discharge plans

• Make appropriate referrals to community resources and support services

• Collaborate with physicians, nurses, case managers, and interdisciplinary teams

• Maintain accurate and timely documentation

• Advocate for patient needs and access to services

• Assist patients and families with coping strategies and care transitions

Required Qualifications

• Master's Degree in Social Work (MSW)

• Active Licensed Master Social Worker (LMSW) License issued by NYSED

• Primary Source Verification of LMSW License

• Minimum 1 year of LMSW experience

• Strong assessment, counseling, and discharge planning skills

• Excellent communication and interpersonal abilities
