Cuready Healthcare Staffing is seeking a qualified RN, Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.
\n\nResponsibilities\n

- Coordinate care for patients post-heart transplant, including immunosuppression monitoring and antibiotic management.
- Monitor patients for signs of organ rejection and infection, ensuring timely intervention.
- Communicate effectively with multidisciplinary team members, patients, and families.
- Facilitate patient activities and coordinate with consulting departments and physicians.
- Assess and address educational and support needs of patients and families.
- Adapt to and coordinate care within UCSF facilities, ensuring compliance with protocols.

- Valid CA RN License
- AHA BLS certification
- Minimum of 1 year experience in ambulatory care
- Experience in cardiac nursing and management of heart failure or heart transplant patients
- Proficiency in Epic charting system
- COVID-19 and flu vaccination compliance
