Job DescriptionPosition Description: Position Summary
Under the general supervision of the Programs & Services Director, the Social Services Associate is part of a multidisciplinary team providing epilepsy-related outpatient and community-based services across Central and South Texas. This is an in-person position based at the San Antonio office, with required travel to outpatient clinics and community locations as needed. The Social Services Associate focuses on case management, patient support, and coordination of social services to help individuals and families improve quality of life and social functioning and facilitates and coordinates multiple support groups in both in-person and virtual formats. The role works closely with clinical staff, community partners, and external agencies to support comprehensive patient care.
Essential Duties & ResponsibilitiesProvide case management services to patients, clients, and families, including needs assessments, service planning, and referrals
Conduct Social Determinants of Health screenings and PHQ-2 screenings
Complete initial meetings or phone calls with new clinic patients and individuals seeking social services
Review and process new patient applications and eligibility documentation
Screen clients referred for emergency medication assistance in collaboration with the Clinical Services Manager
Attend outpatient epilepsy clinics as required to support patient care and interdisciplinary collaboration (Austin, Corpus Christi, Eagle Pass, Laredo, San Antonio, and Weslaco)
Advocate for and empower patients and clients through education, guidance, and support
Serve as a liaison between patients, healthcare providers, and community agencies
Provide crisis intervention services as needed
Participate in post-clinic reviews and interdisciplinary team meetings
Facilitate and coordinate multiple support groups, including in-person and virtual formats
Coordinate the San Antonio Counseling Program
Complete HHSC quarterly client reporting in collaboration with the Clinical Services Manager and Programs & Services Director
Deliver culturally competent services in accordance with organizational standards
Follow all EFCST policies and procedures, as well as Department of State Health Services policies related to epilepsy services
Participate in EFCST events and activities, including occasional evenings and weekends
Perform other duties as assignedEducation & KnowledgeBachelors degree in Social Work, Human Services, or a related field, or equivalent experience
Knowledge of community resources and public/private social service systemsExperience, Skills & AbilitiesMinimum of three (3) years of experience as a case manager or social services professional
Experience working with individuals with disabilities
Strong documentation and organizational skills
Ability to motivate and support individuals toward achieving goals
Clear understanding of professional boundaries and confidentiality
Ability to work effectively with culturally diverse individuals, families, and communities
Ability to work independently while functioning as part of a team
Communication & Professional SkillsSpanish fluency (reading, writing, and speaking) is required
Strong verbal and written communication skills
Proficiency with computers and electronic documentation systems
Ability to communicate professionally with patients, families, staff, and community partners
Public speaking may be required
Must consistently represent the Foundation in a professional mannerDecision-Making AuthorityMakes independent decisions related to patient/client care within established policies and guidelinesPhysical Requirements & Work EnvironmentAbility to work effectively in routine and emergency situations
Ability to lift or move materials and equipment as needed
Regular in-person work is required at the San Antonio office, with duties performed in office, clinic, and community-based environments
Subject to normal conditions associated with medical clinic operationsTravel & Additional RequirementsTravel to outpatient clinics and community locations is required as part of the role
May occasionally drive a 15-passenger van
Must possess a valid drivers license, current automobile insurance, and reliable transportation
Must be able to work evenings and weekends as duties require
May occasionally participate in outdoor activities

$45,000.00 - $48,000.00 Annually
