Job Summary:

The Intake Supervisor is responsible for overseeing a team of Intake Coordinators, ensuring efficient and effective intake processes for members in a community support setting. The role involves conducting comprehensive member intakes and assessments, collaborating with external referral resources, and coordinating activities of the Intake Coordinators. The Intake Supervisor works closely with staff to develop action plans for members, reviews partner portals for authorizations, and maintains accurate records in company platforms. Additionally, the role includes policy development and compliance adherence.

Supervisory Responsibilities:

  • Oversees a team of Intake Coordinators at various sites throughout the state.
  • Provides constructive and timely performance evaluations.

Duties/Responsibilities:

  • Conducts and creates comprehensive member intakes and assessments, ensuring a thorough understanding of individual needs and challenges.
  • Acts as liaison with external referral resources and programs to create and maintain systems of member intakes.
  • Manages the intake team's time cards in designated platform, as well as corrective action pertaining to attendance.
  • Create and provide oversight for Intake Coordinator activities.
  • Collaborates with staff to develop detailed plan of action for members to achieve goals.
  • Review MCP partner portals for new authorizations and input relevant notes pertaining to the member's case.
  • Assist enhanced case managers and members in exchanging resources and referral information for holistic support.
  • Collaborate with Serene Community Support Manager and Directors to administer member changes and updates, including case assignments.
  • Prepare and enter all relevant data into company platforms to maintain accurate and up-to-date records.
  • Stay informed about company requirements and policies, ensuring compliance and alignment with organizational standards.
  • Responsible for developing policies and procedures for the Intake department and generating reports and creating agendas.
  • Ability to interact professionally with all Health Plan partners.
  • Designs and implements systems for Intake Coordinators to enhance outcomes for high-risk patients.
  • Follows CS rules and regulations to ensure ongoing program compliance.
  • Performs other duties as assigned.

Education and Experience:

  • Bachelor's degree in a related field (Social Work, Psychology, Human Services, etc.), or work experience in lieu of education.
  • Previous experience in a supervisory or manager role, preferably in community support services or healthcare.

Required Skills/Abilities:

  • Proven experience in intake management, preferably in a community support setting.
  • Strong organizational and multitasking skills with attention to detail.
  • Excellent communication and interpersonal skills to engage effectively with members and internal teams.
  • Proficient in using company platforms and databases for data entry and information management.
  • Ability to work collaboratively in a dynamic and fast-paced environment.

Physical Requirements:

  • Willingness to travel to different locations as needed.
  • Prolonged periods sitting at a desk and working on a computer.
  • Must be able to lift up to 15 pounds at times.

Pay range$29—$32.50 USD
