Qualicare Home Health is hiring a Per Diem Physical Therapist to provide care within Alameda County with geographically optimized scheduling designed to reduce drive time and support work-life balance.
We focus on assigning patients within a primary coverage territory whenever possible to help create a more manageable and efficient schedule.

Primary Coverage Areas
Patients are assigned based on scheduling efficiency and preferred territory whenever possible.
Typical coverage may include:
- Oakland
- Berkeley
- Hayward
- Fremont
- Surrounding Alameda County communities
Responsibilities
- Perform patient evaluations and follow-up visits
- Develop individualized care plans
- Provide skilled therapy interventions
- Educate patients and caregivers
- Collaborate with interdisciplinary care teams
- Complete accurate and timely documentation
Qualifications
- Active California PT license
- Home Health experience preferred for Per Diem
- Strong clinicians transitioning into Home Health are encouraged to apply
- Excellent communication and organizational skills
