Physical Therapist (PT) – Per Diem | $100–$120/Visit | Alameda County

Qualicare Home Health is hiring a Per Diem Physical Therapist to provide care within Alameda County with geographically optimized scheduling designed to reduce drive time and support work-life balance.


We focus on assigning patients within a primary coverage territory whenever possible to help create a more manageable and efficient schedule.

Patients are assigned based on scheduling efficiency and preferred territory whenever possible.

Responsibilities

  • Perform patient evaluations and follow-up visits
  • Develop individualized care plans
  • Provide skilled therapy interventions
  • Educate patients and caregivers
  • Collaborate with interdisciplinary care teams
  • Complete accurate and timely documentation


Qualifications

  • Active California PT license
  • Home Health experience preferred for Per Diem
  • Strong clinicians transitioning into Home Health are encouraged to apply
  • Excellent communication and organizational skills
