In This Role, You Will:

  • Provide safe, accurate, patient-centered care in collaboration with interdisciplinary teams.
  • Educate and empower patients and families to foster confidence in care plans and support positive outcomes.

Qualifications:

  • Active RN license in the state of employment with a minimum of one year of clinical experience delivering quality care.
  • Valid driver’s license and the ability to travel to patient locations (approximately 20% of the day).
  • Strong time management skills, proficiency with technology (EMR, mobile devices), and a commitment to timely documentation.

Compensation includes payment related to patient care visits, non-visit activities, paid time off, shift premium pay, and the first-year success bonus.
