About the Role
Guardian Angel Home Care is seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to join our growing home health team. In this role, you will provide high-quality, patient-centered care in the comfort of patients’ homes while working alongside a supportive interdisciplinary team.

Responsibilities
-
Provide skilled nursing care in patients’ homes
-
Monitor patient condition, vital signs, and overall health status
-
Administer medications and treatments as ordered by the physician

Accurately document all visits and care provided

Communicate changes in patient condition to the RN/physician
-
Educate patients and families on care plans, medications, and disease management
-
Ensure compliance with all state, federal, and agency regulations

Collaborate with the interdisciplinary team to ensure optimal patient outcomes
Requirements

Active California LVN license in good standing
-
Graduate of an accredited practical nursing program

BLS certification
-
Strong clinical, communication, and documentation skills
-
Ability to work independently in a home health setting
Preferred Qualifications
-
Home health or hospice experience
-
Experience with Homecare Homebase (HCHB) or other EMR systems
-
IV therapy, wound care, or other specialty certifications
-
Bilingual is a plus
