Clinical Pharmacist Opportunities – Terre Haute, IN
Direct Hire | Full-Time | Day Shift
A not-for-profit healthcare system in Terre Haute, Indiana is hiring Clinical Pharmacists across multiple inpatient and outpatient care environments. These are permanent, benefits-eligible roles offering competitive compensation, a $20,000 sign-on bonus, predictable day schedules, and long-term career growth within an integrated health system.
Available Clinical Pharmacist Roles
1. Transition of Care – Clinical Pharmacist (Acute Care Hospital)
• Full-time, day shift: 7:30a–5:30p
• No weekends or holidays
• Based in an acute care hospital with a strong focus on discharge planning
• Emphasis on medication reconciliation, discharge prescriptions, and patient education
2. Clinical Pharmacist – Regional Hospital / Ambulatory Services
• Full-time, day shift (varied daytime schedule)
• Based in a regional hospital campus with robust outpatient and ambulatory services
• Focus on chronic disease management, anticoagulation services, and interdisciplinary clinical care
3. Clinical Pharmacist – Medical Group / Outpatient Clinics
• Full-time, day shift: 8:00a–5:30p
• Embedded within a multi-specialty physician group and outpatient clinic network
• Emphasis on provider collaboration, medication optimization, and patient-facing clinical services
Compensation & Incentives
• Hourly range: $53.37 – $86.48/hr (DOE, role-dependent)
• Annualized range: approximately $111K – $179K
• $20,000 sign-on bonus (paid at hire with a 2-year commitment)

Key Responsibilities (Role-Dependent)
• Review and process discharge medications
• Provide medication counseling and patient education
• Perform drug information and dosing consultations
• Participate in interdisciplinary committees (P&T, Antimicrobial Stewardship, Sepsis, Medication Safety, etc.)
• Support chronic disease management through collaborative drug therapy management (CDTM) protocols
• Precept pharmacy students and residents
• Maintain accurate documentation in EPIC
Requirements
• PharmD from an accredited program
• Licensed or eligible for licensure in Indiana
• 1–2 years of residency or equivalent clinical experience preferred
• Board certification preferred within the first year of eligibility
• Experience in acute care, ambulatory, or transitions-of-care settings preferred (depending on role)
