$25–$29/hour (based on experience)
Position Summary:
We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to provide one-on-one skilled nursing care for a 47-year-old male patient with Cerebral Palsy and Seizure Disorder. The patient maintains an oral regular diet and ambulates with assistance. Care is provided in the patient’s home in Wickliffe, Ohio.
The ideal candidate will have at least one (1) year of skilled nursing experience, a current Ohio LPN license in good standing, and a dedication to delivering high-quality, patient-centered care.
Key Responsibilities:

Provide skilled nursing care in accordance with the Ohio Nurse Practice Act and individualized care plan.

Monitor and document patient condition, including seizure activity.

Assist with mobility, transfers, and safe ambulation.

Support activities of daily living as needed.

Maintain HIPAA compliance and follow infection control protocols.

Collaborate with family and healthcare team to support care goals.

Administer medications as prescribed.
Minimum Qualifications:

Active LPN License in Ohio (unencumbered).

One (1) year of skilled nursing experience in:

Home health with high-acuity patients
-
Long-term care or skilled nursing facility
-

Current CPR/BLS certification.

Must pass background check and drug screening.
Preferred Qualifications:

Experience with seizure disorders.

Familiarity with electronic charting systems.
Work Environment:
Care is provided in a home setting and may require standing, lifting, and assisting with transfers. Compliance with PPE and safety protocols is mandatory.
