Home Care Registered Nurse Compensation

• Pay rate: 56.14 per hour
• Mileage Reimbursement: 0.43/mile
• 6am- 2;30pm Mon - Fri.
• County Covered: Allen, Barren, Hart
  Preferred Skills: HCHB experience preferred, wound care, IV's

Home Care Registered Nurse Responsibilities:

• Perform comprehensive assessment of high-risk members.
• Collaborate with primary care providers to implement individualized care plans.
• Address and reassess cost-efficient levels of care.
• Coordinate care transitions as needed.
• Engage specialty resources to achieve optimal resolution.
• Act as patient advocate by protecting privacy and confidentiality.

Home Care Registered Nurse QUALIFICATIONS:

Critical Information

• Shift: Monday-Friday 8am-5pm, on call every 3rd weekend and 1-2 nights during the week.
• On-site position.

Education/Licenses Needed

• Current, unrestricted Registered Nurse license with compact privileges.
• Associates or Bachelor’s Degree in Nursing or related field.
• Case Management certification.
