- Educate patients and families on diagnoses, medications, follow-up care, and health management to ensure successful discharges and continuity of care.
- Play a significant role in discharge planning and care coordination, ensuring patients and caregivers understand their next steps and have the necessary resources.
- Maintain strict adherence to infection prevention, safety, and quality standards, complying with regulatory requirements and facility policies.
Requirements and Qualifications
