Job DescriptionWe are looking for a Home Care Scheduling Coordinator to help manage our Senior Care agency! This role makes a significant impact on our daily operations and the ability to provide the best care to patients by coordinating client services. As the Scheduler, your main responsibilities are to match clients’ requested shifts with caregivers’ personalities and skills, and to maintain the highest levels of client satisfaction and caregiver relations. RESPONSIBILITIES: Essential job duties for this position include the following items. Other duties must be performed as assigned or required.• Use web-based scheduling software as the basis for all scheduling• Track and manage caregivers’ availability and overtime status.• Update schedules in software system• Update software system with client and caregiver notes as needed.• Research and brainstorm solutions to service problems, complaints, and scheduling gaps.• Follow-up on client and caregiver issues to make sure they are resolved.• Schedule client visits according to Service Plan/Care Plans and staff availability• Staff clients’ requests, matching the needs and preferences of the clients with the skills and personalities of the caregivers.• Create and maintain schedules for all caregivers• Contact caregivers regarding shift coverage and any changes to the hours of their regular schedule• Maintain proper staffing levels to ensure proper coverage for all clients• Fill open shifts and securing coverage for call-offs or changes in client's needs• Maintain ongoing communication with Managers, Caregivers & Clients regarding scheduling and changes• Participates in determining hiring needs• Enter scheduling data, create and release weekly schedules for all assigned caregivers• Maintain call-in, retention reports• Discern client services required as outlined in agreements, urgent requests and care plans• Enter client information into a database• Maintain client database (client admission, client hospital admission and discharge)• Perform general office duties including but not limited to word processing, photocopies, files, shreds, sort/distribute mail, provide reception coverage, telephone services, etc.• Share after hours on-call duties to resolve evening and weekend scheduling issues• Work toward continuous quality improvement• Stay current with changing technology, including software programs• Communicate effectively with clients and their families, caregivers, office staff, managers, and the public.• Uphold, support, and promote all company policies and procedures• Adhere to all HIPAA regulations • Monitor missed clock-ins and clock-outs.• Develop proficiency in software system. QUALIFICATIONSHigh school diploma or GED1-2 yrs patient scheduling experienceMust be well organized, show great attention to detailProficient with computer use and software applicationsMulti-tasker and customer service-orientedExcellent time management skillsHome Care experience preferred
