RN Case Manager Hours
- Shift Requirements: M-F 8am-5pm
- ON Call Required- Every 3rd or 4th week/weekend
- Counties covered: Daviess, McLean, Ohio, Webester, Henderson
RN Case Manager Compensation
- The pay for this position is $50.00-55.00 an hour + mileage reimbursement
- Benefits are available to full-time employees
- A 401(k) with a company match is available for full-time employees.
RN Case Manager Responsibilities
- Perform comprehensive assessments of high-risk members
- Develop and implement individualized, comprehensive care plans in collaboration with primary care providers
- Coordinate care transitions and ensure appropriate levels of care
- Identify and address non-medical barriers to care (e.g., housing, transportation)
- Collaborate with physicians, social workers, discharge planners, and claims professionals
- Engage specialty resources as needed to achieve optimal patient outcomes
- Monitor clinical, functional, and fiscal outcomes and maintain detailed documentation
- Identify opportunities for health promotion and illness prevention
- Prevent adverse patient events when possible and intervene promptly when necessary
- Act as a patient advocate while ensuring privacy and confidentiality
- Ensure compliance with hospice standards and organizational policies
RN Case Manager Requirements
- Licensed RN
- HCHB, Home Health Experience 1 -year
- IV, Wound care, Wound vac, etc
