Position Summary
The Licensed Practical Nurse (LPN) provides direct nursing care and supports behavioral health services for residents in a 16-bed Enhanced Services Facility (ESF). Under the supervision of the Registered Nurse (RN) and in accordance with acute inpatient treatment and health & safety standards, the LPN assists with medication administration, vital sign monitoring, documentation, and resident health assessments. The LPN ensures adherence to established care plans and contributes to maintaining a safe, therapeutic, and recovery-focused environment.

Classification: Non-Exempt
Schedule: Full-time, Part-time, or Per Diem (Day, Evening, or Night Shifts)
Compensation: $32.00 - $42.00 per hour
Core Duties and Responsibilities

Provide nursing care and medication management ensuring adherence to each client’s Person-Centered Service Plan (PCSP).

Support implementation and documentation of the Behavior Support Plan (BSP) and Crisis Prevention and Response Protocol (CPRP) as required by DSHS.

Collaborate with the RN and Program Coordinator during assessments and team reviews.

Maintain readiness for DSHS, RCS, and DOH inspections and audits.

Participate in Quality Improvement (QI) and Continuous Quality Improvement (CQI) activities.

Assist with staff training, documentation audits, and infection control measures.

Support the RN in crisis response, medication safety, and coordination of health-related services.

Participate in emergency response, evacuation, and infection control drills.

Follow all infection prevention, PPE, and sanitation protocols.

Maintain readiness for emergency situations, including behavioral and medical crises.
Qualifications
-
Current Washington State Licensed Practical Nurse (LPN) license in good standing.

Minimum one year of experience in psychiatric, behavioral health, or long-term care.

Knowledge of medication administration, infection control, and documentation standards.

CPR/First Aid certified; Therapeutic Options certification preferred.

Strong communication, assessment, and teamwork skills.

Familiarity with ECP or equivalent EMR system.
Essential Physical Requirements
-
Must be able to lift and/or move up to 50 pounds.

Must be able to stand or walk for extended periods and assist residents with mobility.

Must have the physical and mental endurance to manage stressful or crisis situations calmly and effectively.
