Physical Therapist (PT) / Physical Therapy Assistant (PTA) – Home Health (Per Diem)
Location: Boca Raton, FL

We are seeking a Per Diem Physical Therapist (PT) or Physical Therapy Assistant (PTA) to join our Home Health team in Boca Raton, FL. This role offers a flexible schedule and the opportunity to provide high-quality rehabilitative care in a home setting.
Responsibilities:
- Conduct in-home patient evaluations and implement individualized treatment plans (PTs).
- Assist in executing prescribed therapy programs under the supervision of a licensed PT (PTAs).
- Provide therapeutic exercises and interventions to improve mobility, strength, and overall function.
- Educate patients and caregivers on home exercise programs and safety measures.
- Collaborate with an interdisciplinary team to ensure optimal patient outcomes.
- Accurately document patient progress and treatment plans in an EMR system.
- Maintain compliance with state, federal, and agency guidelines.
Requirements:
- Active Florida PT or PTA license.
- Home Health experience preferred.
- Familiarity with OASIS documentation and EMR systems (such as HomeCare HomeBase or Kinnser WellSky) is a plus.
- Strong interpersonal and communication skills.
- Ability to work independently and manage a flexible schedule.
- Reliable transportation for patient visits.
