Key Responsibilities
Optimal Care Home Health Clinical Managers are Registered Nurses who manage the coordination, supervision, and implementation of professional and supportive services to clients in a safe, cost-effective manner. The Clinical Manager may assist in education and training related to the clinical practice, regulation, and reimbursement changes.
In this role you will be responsible for:
- Provides expert guidance and assistance in all matters relating to personnel and patient care services.
- Responsible for providing oversight and helping to manage patient and personnel assignments to ensure optimal management of the complex and interdisciplinary care of home health patients
- Responsible for ensuring coordination of care for patients through development and implementation of policies and procedures, personnel education and in-services, and assisting in care plan development with clinical staff
- Responsible for ensuring coordination of referrals for patients
- Responsible for ensuring the assessment of patient needs and ensuring the plan of care is developed, implemented, and updated
- Assists in the development and interpretation of the philosophy and objectives of the agency
Required Qualifications
- Bachelor's degree in Nursing
- Current state license as a Registered Nurse
- 2+ years experience as a Registered Nurse
- 2+ years experience in home health
- Nursing skills as defined by accepted nursing standards
- Knowledge of business management, governmental regulations, and CHAP standards
- Demonstrates ability to assess and respond to the needs of patients, families, and caregivers in varied settings and to function effectively in the home or nursing facility environment
- Demonstrates skill in nursing practice, ability to cope with ill, disabled, terminal patients, family emotional stress, and tolerance of individual lifestyles
- Demonstrates ability to handle multiple priorities, documentation requirements, and deadlines
- Current and valid driver's license
- Reliable transportation and valid automobile insurance coverage
