Location: Hybrid - travel to member homes / facilities. Candidates should reside near Albuquerque, NM
The Community Outreach Specialist supports members who are difficult to reach by conducting telephonic outreach and in‑person home visits to complete Health Risk Screenings (HRS) or Health Risk Assessments (HRA). This role plays a critical part in connecting members to healthcare programs and social services while supporting the Social Services and Physical & Behavioral Health Management teams.
Key Responsibilities
- Conduct telephonic outreach and home visits to engage members identified as unable to reach
- Complete Health Risk Screenings (HRS) and Health Risk Assessments (HRA) during member interactions
- Perform 5-7 home visits per day, traveling within a 50‑mile radius of home residence
- Educate members on available healthcare programs, benefits, and services
- Respond to member inquiries and provide assistance with healthcare‑related needs
- Collaborate with community partners and internal teams to support member engagement
- Support clinicians in the Physical and Behavioral Health Management department with member and provider activities
- Perform ongoing functions that support the Social Services Department
- Accurately document member interactions and follow established procedures
Job Requirements
- Bachelor’s degree OR 4 years of experience in healthcare, social services, or community advocacy
- Knowledge of medical terminology
- Experience coordinating member needs and providing member assistance
- Ability to assess and analyze member needs
- Strong customer service and communication skills
- PC proficiency, including Microsoft Office applications
- Must have a reliable vehicle and valid auto insurance
- Ability to travel daily within assigned service areas
Preferred Qualifications
- Community Health Worker (CHW) certification from an HCSC‑approved college
- Bachelor’s degree in Social Work
