Job DescriptionAbout the Role:Join Guardian Residential Services LLC as a Skilled In-Home LPN for the Night Shift, providing dedicated one-on-one care to our clients in Manassas, VA. Responsibilities:

• Provide compassionate, high-quality nursing care to clients in their homes during the night shift.
• Administer medications and treatments as prescribed by healthcare providers.
• Monitor and document patient health status and vital signs.
• Assist with daily living activities, including personal hygiene and mobility.
• Communicate effectively with clients, families, and healthcare teams.
• Maintain a clean and safe environment for clients.
• Educate clients and families on health management and wellness strategies.
• Respond promptly to client needs and emergencies.

Night Shift LPN Skilled Nursing Responsibilities and Care Competencies:

Routine GT-Tube Care and Overnight Feeding Management: Monitor placement and patency of the G-tube site throughout the night, assess for leakage or redness, ensure secure tubing, and administer scheduled or continuous feedings as ordered. Document tolerance, intake amounts, and any signs of aspiration risk.

BiPAP/CPAP Monitoring and Support: Ensure equipment is functioning properly, monitor mask fit to prevent air leak or skin breakdown, observe respiratory effort while asleep, and record settings and overnight respiratory quality. Notify on-call nursing or parent/guardian of any concerns as required.

Catheter Care and Bowel Management: Perform sterile straight catheterizations or maintain indwelling catheters, ensuring proper positioning, drainage, and cleanliness to prevent UTI or skin irritation. Implement bowel management protocols, record output, and monitor patterns or signs of discomfort.

Respiratory Therapy Support (Chest Physiotherapy/Cough Assist): Provide scheduled chest PT and cough assist therapy at designated intervals to promote airway clearance. Monitor oxygen saturation and work of breathing during and after treatment, documenting tolerance and outcomes.

Continuous Pulse Oximetry and Overnight Safety Monitoring: Maintain continuous pulse ox equipment throughout the night, respond promptly to alarms, evaluate for desaturation events, adjust positioning, and escalate concerns per protocol. Ensure monitoring aligns with the family’s/guardian’s established standard of care and support expectations.

Requirements:

• Current LPN license in the state of Virginia.
• Minimum of 1 year of nursing experience, preferably in home health or long-term care.
• Strong communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Compassionate demeanor and commitment to patient-centered care.
• Basic Life Support (BLS) certification required.
• Willingness to work night shifts and flexible hours.
• Reliable transportation to travel to client locations.
