Job Description
Case Planner Needed ($65,000/year)
Workforce1 Healthcare Career Center is partnering with top NYC healthcare providers to connect qualified Case Planner to immediate openings at a healthcare organization within the Bronx.
Key Responsibilities:
<ul>
<li>Guide families through a structured, three-phase treatment model: Initial, Baseline, and Stabilization.</li>
<li>Conduct comprehensive assessments and develop individualized service plans.</li>
<li>Coordinate multidisciplinary interventions tailored to each family's unique needs.</li>
<li>Build meaningful relationships with families over an average case duration of 12 months.</li>
<li>Collaborate with internal and external providers to ensure safety, stability, and well-being.</li>
</ul>
Minimum Qualifications:
<ul>
<li>BSW or bachelor's degree in human services REQUIRED.</li>
<li>Some knowledge of the child welfare system.</li>
<li>Fluency in English and Spanish a Plus</li>
<li>Citywide travel required- 14 cases up to 4 visits with each family per month</li>
