Home Health RN Contract Assignment in OFallon, IL

Sunbelt Staffing is hiring an experienced Home Health RN for a contract position based in OFallon, IL. This is a full-time opportunity for a nurse comfortable with field-based care and autonomous patient management.

Key Responsibilities:

• Deliver skilled nursing care to patients in their homes throughout the OFallon area

• Complete SOC, recert, discharge, and routine OASIS visits

• Maintain accurate documentation using HomeCare HomeBase (HCHB)

• Manage a caseload of 46 points per day

Schedule:

• Monday through Friday

• 8:00 AM 5:00 PM, 40 hours per week

Qualifications:

• Active Illinois RN license

• Minimum of 1 year recent home health experience

• Strong working knowledge of OASIS and HCHB

• Reliable transportation required
