Daughters of Israel is a non-profit skilled nursing facility. We are currently seeking a Director of Social Services to oversee and ensure that the medically related emotional and social needs of the residents are met on an individual basis.

Responsibilities:

  • Oversee all aspectes related to the social services department to ensure all the emotional and social needs to the residents/patients are met.
  • Work with the IDT team to develop and implement policies and procedures for the identification of medically related social and emotional needs of the patient/resident.
  • Participate in planning, development and implementation of care plans, resident assessments, and discharge planning.
  • Interview patient/resident/families as necessary and in a private setting. Involve the patient/resident/family in planning objectives and goals for the patient/resident. Develop preliminary and comprehensive assessments of social service needs of each patient/resident.
  • Assist and develop a written plan of care for each resident that identifies social problems/needs of the patient/resident and the goals to be accomplished for each problem/need area.
  • Refer resident/patient/family to appropriate social service programs, as needed.
  • Review Advance Directives with patients/residents, provide information on Advance directives and care plan accordingly.
  • Participate in regularly scheduled Utilization Reviews of residents/patients and review discharge plans and barriers to discharge.
  • Perform administrative requirements such as completing necessary reports, approval of payroll and scheduling, etc.
  • Provide case management and information to the resident/patient/families as to Medicare/Medicaid and other financial assistance programs available to the resident/patient and refer to business office when appropriate.
  • Maintain a quality working relationship with the medical professionals and other health related facilities and organizations that work with DOI during the admission and discharge process.
  • Make written and oral reports/recommendations to the IDT & QAPI Teams concerning the operation of the social service department.

Requirements/Experience

Licensed Clinical Social Worker (LCSW) preferred. LMSW and MSW will be considered depending on years of management/director experience.

Direct management experience in long term / short term rehabilitation environment required
