We are currently seeking a skilled and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our innovative VBC team. Essential Job Functions:What Youll Do As an integral member of our VBC team, youll: Provide Direct Patient Care: Conduct Annual Wellness Visits (AWVs) focused on prevention, risk assessment, and advance care planning for Medicare patients. Manage chronic and acute conditions through evidence-based care plans. Provide evaluation and management of complex wounds, including diabetic ulcers, pressure injuries, surgical wounds, and skin grafts. Perform or coordinate advanced wound care procedures and treatment plans, including debridement, wound dressing selection, and infection prevention. Lead efforts in the ongoing monitoring and management of patients with chronic or non-healing wounds. Close care gaps with focused visits that address quality metrics and improve health outcomes. Coordinate and Lead Care Efforts: Triage patient needs via phone and electronic messages. Collaborate closely with primary care providers, specialists (e.g., podiatry, dermatology), care managers, and pharmacists. Guide and support medical assistants and care coordinators in clinical and wound care efforts. Drive Quality and Performance: Monitor and improve key performance indicators such as HEDIS and Stars metrics. Participate in strategic meetings to optimize workflows and care models. Lead efforts in patient outreach, population risk stratification, and process improvementincluding for high-risk patients with wound care needs. Ensure Excellent Documentation and Compliance: Maintain thorough and timely documentation in the EMR, especially related to wound assessments and care plans. Support accurate coding and regulatory compliance for both general VBC requirements and specific wound care procedures. Empower and Educate Patients: Provide personalized education on managing chronic illnesses, medications, wound prevention, and self-care techniques. Encourage patient engagement and shared decision-making to support long-term healing and improved quality of life.
Qualifications:What Were Looking For Education & Licensure: Masters or Doctorate in Nursing (NP) or completion of an accredited PA program. Unrestricted Utah license and national certification (AANP, ANCC, or NCCPA). DEA licensure and BLS certification required; ACLS preferred. Wound care certification (e.g., WCC, CWCN, or equivalent) preferred or willingness to pursue. Experience: Minimum 2 years as an APP preferred. Background in primary care, geriatrics, or chronic disease management strongly preferred. Experience or demonstrated interest in wound care, skin grafts, and ulcer management highly preferred. Value-Based Care or population health experience a major plus. EMR proficiency required; experience with systems like Epic, Cerner, or Athenahealth is ideal. Key Skills: Clinical excellence in assessment, diagnosis, treatment, and wound management. Strong communication and patient education abilities. Team-oriented with leadership capabilities, especially around wound care coordination. Deep understanding of value-based care and quality metrics.
Hours:8 AM to 5 PM Monday to Friday
