The Outpatient Case Manager ensures a seamless experience for clients from intake through ongoing care, providing person-centered support and coordination across services. Pay:$57,000 - $62,000 Annually
Shift: Full-Time, 1st shift

This job is expected to complete the following functions. This is not an all-inclusive list. This job may be assigned other functions or tasks as business needs dictate. This is meant to be a general description of the most essential duties only.

  • Conduct brief biopsychosocial assessments to determine program fit and level of care

  • Build rapport with clients, families, and referral sources to support engagement

  • Coordinate admissions, including verifying funding, arranging transportation, and addressing barriers to care

  • Initiate and complete documentation (e.g., progress notes, treatment plans, group notes) within required timeframes

  • Monitor and manage outpatient census and waitlists

  • Support clients in navigating external systems (housing, financial aid, transportation)

  • Collaborate with multidisciplinary teams to develop and implement individualized treatment goals

  • Provide direct case management support to patients as needed

  • Maintain accurate records and ensure warm hand-offs to appropriate care teams

Required SkillsRequired:

  • Bachelor’s degree in Social Work, Human Services, or related field
  • Valid license (LSW, LGSW, LISW, LICSW, LADC) or 4+ years of case management experience

  • Experience with co-occurring disorders and chemical dependency

  • CPR certification (or ability to obtain prior to hire)

  • Valid MN driver’s license

  • Must be free of substance use problems for at least one (1) year immediately preceding their hiring and must sign a statement attesting to that fact.

  • Strong verbal and written communication skills

  • Ability to communicate with diverse populations

  • Knowledge of the 12 core functions and systems theory

  • Organizational skills and attention to detail

  • Flexibility to manage changing priorities in a fast-paced environment

  • Proficiency in documentation and communication tools

  • Ability to work independently and collaboratively

Preferred:

  • Experience in outpatient or residential substance use treatment

  • Background in sales, marketing, or business development


Physical Requirement  

  • Hearing Ability to hear staff, patients, incoming calls, and callers.

  • Vision Ability to see computer screen, paper documents, patient charts

  • Hearing Will experience moderate noise levels

  • Speaking Ability to speak to staff and external customers/vendors

  • Sitting Up to 6 hours per day

  • Stand/Walking Up to 2 hours per day

  • Lifting Up to 15 Ibs

  • Travel Up to 15% by car
