Job Summary: The Case Coordinator provides supervision to frontline Behavioral Interventionist (BI) staff and case management to families served in the BI Program.

Responsibilities: Primary responsibilities include but are not limited to:



Case Management

  • Responsible for family satisfaction within the BI program. Ensure families served are given proper support, advocacy, resources, and communication in a timely manner and implementing trauma informed care practices.
  • Complete required assessments with families in a timely manner, scoring correctly, tracking client progress, and utilizing assessments to adapt services to the needs of the client and family.
  • Attend court appointments and other professional family-based meetings as required.
  • Work with team in matching BIs to homes, resolving issues in a timely manner, and participating and engaging in team meetings.
  • Review Incident Reports with Program Manager and follow-up with family, if needed.
  • Complete & facilitate Referral Triage, Intake, and Meet & Greets for each family within the required time frames.
  • Complete and submit approved Recommendation and Approval Letters for services in a timely and professional manner.
  • Create Behavioral Plan for each client, obtaining approval and signatures in a timely manner and review and update as necessary.
  • Keep families and professional team members updated and informed when placed on waitlist and during the process of Admission Meeting, approval, and BI hiring process.
  • Complete all Quarterly Meetings and Bi-annual Assessments with each family and if applicable, with the BI, within the required time frame.
  • Write and submit renewals in a timely manner.
  • Update SAM, electronic health record, profiles of any and all changes in the required time frame.
  • Complete End of Month Reports and turn in to supervisor by the 1st of the following month.
  • Complete Case Closure and Aftercare Follow-up meetings, assessments, and documentation in the required time frame.
  • Review BI Quarterly Progress Reporst in a timely manner and provide feedback to staff.

Administrative Leadership & Supervision

  • Responsible for employee (BI) satisfaction within the In-Home Behavioral programs. Strive to build a strong sense of team within the department and implementing trauma-informed care practices.
  • Ensure frontline staff are given proper support and advocacy.
  • In collaboration with the Hiring Coordinator and Program Manager, conduct interviews, and participate in the on-boarding process of Behavioral Interventionists.
  • Assist in planning and executing BI monthly meetings.
  • Conduct client/staff matchings in collaboration with the Program Manager.
  • Conduct individual phone supervisory conferences at least monthly, and face-to-face conferences quarterly, with the goal of on-going evaluation of the Behavioral Interventionists work product. Any noted concerns (i.e. work performance or client issues) need to be communicated with the Program Manager within twenty-four

(24) hours of report.

  • Complete bis 90-day and annual evaluation processes.
  • Provide in-home observation shifts of BIs every six (6) months and provide timely feedback.
  • Track all missed shifts and check shift notes daily for completion.
  • In partnership with the Program Manager, complete necessary corrective actions and terminations.
  • Update and track employee timecards in the payroll system.
  • Responsible for providing support to BIs during OHI Investigations.
  • Participate as an active team member with colleagues of all demographics in order to effectively and efficiently serve clients.
  • Maintain contact and relationship building with other partner agencies and community stakeholders.
  • Maintain strong, open communication with colleagues and direct supervisor regarding services delivered, concerns or questions.
  • Maintain CPR/First Aid certification.
  • Work collaboratively with branch office(s) to ensure program fidelity.
  • Attend trainings as required for ongoing professional development.

Outcome Planning/PQI

  • Complete agency reports and statistics as needed or assigned.
  • Ensure procedures are in place to track and report program outcomes and quality assurance.
  • Ensure all resource provider files are in compliance with requirements.
  • In partnership with the Program Manager, complete Critical Incident Reports as necessary.
  • Complete all Incident Report Reviews within 2 business days of the Critical Incident Report date of occurrence.

Qualifications & Preferred Skills

  • Masters Degree (or Bachelors Degree plus two (2) years of experience working in non-profit social service agencies) in social work, psychology, public administration, or a related human services field.
  • Previous experience working with at-risk populations required.
  • Familiarity with the child welfare system
  • Demonstrated resourcefulness in setting priorities, proposing new ways of creating efficiencies, and guiding investment in people and systems.
  • Core understanding of program development, implementation and evaluation.
  • Outstanding organizational and planning skills.
  • Solid commitment to the principles of support and advocacy for foster and adoptive families.
  • Strong verbal communication skills and demonstrated ability to write clearly and persuasively.
  • Demonstrated leadership ability, team management, and interpersonal skills. Must have ability to quickly establish rapport and build trust.
  • The ability to collaborate and advocate for positive outcomes, using excellent interpersonal skills in multidisciplinary, diverse, and dynamic professional teams.
  • Flexible and a self-starter; able to multi-task while also being highly detail-oriented while meeting all deadlines.
  • Work as a necessary part of a team effort with colleagues of all demographics.
  • Model ethical and professional behavior at all times.
  • Participate in ongoing meetings/professional memberships and trainings as required by the Program Director/President.
  • Experience working with marginalized communities with a nuanced understanding of systemic oppressions.
  • Flexibility and adaptability, able to shift styles to fit the needs of a wide range of cultures, people, and organizations.
