NOW HIRING!! A-Line is seeking an experienced RN Case Manager for a M-F 1st shift Schedule in Owensboro, KY. The RN Case Manager supports high-risk members through comprehensive assessments, care coordination, and individualized care planning. This field-based role focuses on improving patient outcomes by collaborating with providers, advocating for members, and ensuring efficient, cost-effective care across the continuum.
This is a contract assignment with the opportunity to convert to a permanent position based on attendance, performance, and business needs.

Location: Owensboro, KY 42303
Schedule: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM
Work Environment: 100% On-site, Field-Based
Pay Rate: $56.16/hour

Key Responsibilities
- Perform comprehensive assessments for high-risk members
- Collaborate with primary care providers to develop and implement individualized care plans
- Identify and reassess appropriate, cost-effective levels of care
- Coordinate care transitions across the continuum of care
- Arrange non-medical support services, including housing and transportation
- Partner with physicians, social workers, discharge planners, and interdisciplinary care teams
- Engage specialty and community resources to optimize patient outcomes
- Maintain accurate and timely documentation of clinical, functional, and financial outcomes
- Identify opportunities for health promotion and illness prevention
- Advocate for patients while ensuring privacy and confidentiality
Required Qualifications
- Associate’s or Bachelor’s Degree in Nursing or related field
- Active, unrestricted Registered Nurse (RN) license in the state of practice
- Strong clinical judgment and ability to analyze complex medical information
- Commitment to mandatory attendance during the first 90 days of employment
- Case Management certification preferred
