A healthcare facility in Roseville, CA is seeking a skilled RN with Case Management experience for a contract role to provide comprehensive care coordination and support to patients. This position requires a professional who can effectively manage patient cases, collaborate with interdisciplinary teams, and ensure optimal health outcomes.
Responsibilities:
- Coordinate and manage patient care plans from admission through discharge
- Assess patient needs and develop individualized care strategies
- Facilitate communication between patients, families, physicians, and healthcare staff
- Monitor patient progress and adjust care plans as necessary
- Provide education and support to patients and caregivers regarding treatment plans
- Ensure compliance with healthcare regulations and facility policies
Qualifications:
- Valid RN licensure with demonstrated Case Management experience
- Strong organizational and communication skills
- Ability to work independently and as part of a multidisciplinary team
- Knowledge of care coordination, utilization review, and discharge planning
Schedule:
- Monday to Friday, 08:00 AM to 04:30 PM
