Haven Health, the leading Home Health, Hospice, and Palliative Care company in Southern California, is currently hiring a Nurse Practitioner to join our Palliative Care team.

We provide hands-on training so every Nurse Practitioner is comfortable accomplishing their responsibilities, along with a strong support system where supervisors are readily available to provide guidance.

We also believe education is an important part of a Nurse Practitioner’s growth. We offer an online learning management system so Nurse Practitioners can complete continuing education units at their convenience.

Responsibilities:

• Visit patients in the home and assess their plan of care
• Provide care in your role as a Nurse Practitioner with a focus on quality and comfort
• Submit patient records and documentation in a timely manner
• Collaborate as part of the Interdisciplinary Care Team

Requirements:

Qualifications:

• Master’s Degree in Nursing
• Licensed Nurse Practitioner in the state of California
• Licensed and insured driver with reliable transportation
• Understanding and acceptance of hospice principles of care
• Strong knowledge of medicine, pharmacology, pain and symptom control, and patient admissions criteria
