Key Responsibilities of the Home Health RN
Conduct in‑home patient assessments focusing on safety, environment, and clinical needs
Develop individualized care plans and coordinate services with providers
Administer medications, wound care, IV therapy, and skilled nursing interventions
Provide disease‑management education and support to patients and families
Maintain accurate and timely documentation in EMR systems
Collaborate with PT/OT/SLP, case managers, and interdisciplinary teams
Prioritize daily schedules and manage time and mileage efficiently
Support safe transitions, continuity of care, and discharge planning
Requirements of the Home Health RN
Active Indiana RN license or compact eligibility
Current BLS certification
Minimum 1 year of recent Home Health RN experience
Reliable vehicle and ability to travel between homes
