Position Summary:

Under direction of the Director of Case Management, the Social Services Specialist (formerly titled as Discharge Planner) assists each resident and family in planning discharge for post-SNF stay in order to maintain the highest practicable level of functioning in accordance with current federal and state regulations, as well as Company policies and procedures.

Duties and Responsibilities:

  • Provides assistance to residents and families in finding and utilizing financial and other community resources to assist in the discharge process.
  • Participates in resident and family council meetings as requested.
  • Develops a working knowledge of federal and state regulations, as related to position, and reimbursement (Medicare, Medicaid and commercial) and provides support with applications and eligibility information requests.
  • May perform documentation duties in accordance with Company charting and documentation policies and procedures and government regulations. Ensures documentation is accurate, clear, informative and descriptive of resident's condition, care provided and residents' response to care.
  • Arrangement of transfers to other facilities and post discharge plan of care, as well as orientation for resident and family regarding discharge.
  • Provides support to Admissions by participating in the referral and admission process, as directed.
  • Assists other facility staff with fiduciary matters, to enable payment for resident's stay.
  • Communicates with residents' families and other staff as to resident needs.
  • Participates in and assists in departmental studies and projects as required.
  • May perform administrative duties such as completing forms, reports, etc., and submit to the Supervisor as necessary.
  • Reports the following in accordance with established facility procedures and regulatory standards: accidents and incidents; resident grievances, complaints, allegations of resident abuse and/or misappropriation of resident property; occupational exposure to blood, body fluids, infectious materials, and hazardous chemicals; concerns and suspected incidences of non-compliance including but not limited to hazardous conditions and equipment.
  • Adheres to state and federal regulations and company policies and procedures; HIPAA confidentiality standards of resident and facility information; Infection Control, Universal Precautions, OSHA and safety standards; and established safety precautions and regulations (including use of mechanical lifts when lifting or moving heavy individuals or objects) in the performance of tasks and the use of equipment and supplies.

Qualifications:

  • Previous experience with geriatric population or in long-term care setting strongly preferred
  • High school diploma or equivalent required; AA or Bachelor's degree preferred
