Position Title: Case Manager I
Department: Long Beach Outpatient Mental Health
Reports To: As assigned by Program Director
POSITION SUMMARY
The Case Manager I provides supportive, community-based services to clients living with mental health and co-occurring substance use needs as part of FSP team. This is a field-based position. Under general supervision, this position assists clients in accessing and navigating behavioral health, medical, housing, and social service systems to promote stability, wellness, and independence. The Case Manager I is responsible for delivering strengths-based, culturally responsive services, supporting treatment goals established by clinical staff, and ensuring continuity of care through ongoing coordination and communication with internal and external providers.
This role includes conducting outreach and engagement, gathering psychosocial information, monitoring client progress, and documenting services in compliance with agency, county, and state standards. The Case Manager I work collaboratively with clinicians, peers, and multidisciplinary partners to address barriers related to housing, benefits, transportation, health, and daily living needs. Ideal candidates demonstrate strong organizational skills, empathy, professionalism, and the ability to build rapport with diverse populations.

CATEGORIES OF DUTIES
General Duties:
- Conducts self in a professional manner in areas of dress, language, attitude and behavior.
- Exhibits full knowledge and support of the philosophy and objectives of the Agency.
- Fully understands and maintains policies regarding professional ethics.
- Inform patients about other services offered by Tarzana Treatment Centers, Inc. (TTC) that may be appropriate for the patient’s family members.
- Attends in-services trainings and educational workshops.
- Attends all required staff meetings.
- Cooperates and communicates fully with other staff members regarding patient behavior, progress activities, etc.
- As appropriate works closely with other multi-disciplinary team members to develop a comprehensive Individualized Treatment Plan on assigned patients and to ensure that all identified problems are addressed.
- Assist with employment/vocational referrals, provide vocational counseling re resume preparation, job interviewing etc.
- Provides outreach and follow up services to patients in and out of treatment, to check progress, offer additional services, help reengage in treatment, make needed referrals.
- Facilitates discharge-planning efforts, commencing at time of admission on all assigned patients, including areas of housing, employment, medical psychiatric, financial, aftercare, etc.
- Makes direct contact within agency and with outside entities as appropriate, in order to assist patients in resolution of legal, financial, housing, medical, psychiatric and discharge planning issues
- Provides regular, comprehensive progress note charting in patient’s Medical Record which document contacts, progress towards identified goals, program participation, referrals, discharge planning and other case management activities.
- Performs other duties as assigned by Director and/or Administrator.
EXPERIENCE
1. For AOD Programs:
- Enrollment in an academic Addiction Studies program and registration with CCAPP, CAADE, or CADTP
- 1 year experience in a Behavioral Healthcare setting.
2. For all other Programs:
- 1 year experience in a Behavioral Healthcare setting (HIV programs) and DVSS Certification for DV specific staff
3. Bilingual English/Spanish preferred
4. Valid driver’s license and insurance
LICENSES/CERTIFICATES
For AOD programs, registration or certification with a DHCS approved state level counselor certification agencies.
