Position: Part-Time RN/LPN (24 hours per week, Day Shift 7 AM - 3 PM)
Sign-On Bonus: $3000**
Our organization is in search of a dedicated Part-Time RN or LPN to join our team, working 24 hours a week during the day shift. This role focuses on delivering direct care to residents in a collaborative environment where their well-being is our top priority.

Key Responsibilities
- Carry out professional duties in line with facility policies, standards of practice, and relevant regulations including the State of New Hampshire Nurse Practice Act, OBRA, and HIPAA.
- Work collaboratively with an interdisciplinary team to develop personalized care plans for residents, updating them as needed and participating in care plan meetings.
- Implement care protocols as required, including fall prevention, safety measures, behavior management, weight control, skin care, bowel and bladder management, infection control, and pain management.
- Monitor and document any changes in residents’ physical or mental health, reporting findings to the primary care physician and treatment team to support informed decision-making.
- Provide leadership to direct caregivers, assigning daily tasks and overseeing the quality of care through observation and evaluation.
- Foster positive relationships with team members, residents, and their families to encourage open communication.
- Administer and document medications and treatments in accordance with professional standards and nursing policies, ensuring proper inventory management of narcotics.
- Promote and maintain a positive culture for both team members and residents.
Qualifications
- A minimum of a Nursing Degree or Diploma from an accredited institution is required.
- Experience in a long-term care setting and specialized knowledge in elderly care and rehabilitative nursing is preferred.
- A valid RN or LPN license in the State of New Hampshire is mandatory. Certification in intravenous administration is preferred. Annual CEUs and CPR certification are required.
