Hybrid Case Manager – Youth & Families Hours
- Monday–Friday, 8:00 AM – 5:00 PM (flexibility required to meet member needs)
- Will be able to work remote 1-2 days a week, but 50% of the time will be in the field to patient homes and/or facilities for assessments
- NE Ohio Counties - Candidates applying must live in one of the following counties :
- Ottawa, Erie, Lorain, Cuyahoga, Lake, Ashtabula, Wood, Sandusky, Seneca, Huron, Hancock, Wyandot, Crawford, Marion, Union, Medina, Summit, Portage, Geauga, Trumbull, Mahoning, Stark, Columbiana, Carroll, Tuscarawas, Harrison, Jefferson, Belmont or Monroe.
Hybrid Case Manager – Youth & Families Compensation
- The pay for this position is $27.17 an hour
- Benefits are available to full-time employees
- A 401(k) with a company match is available for full-time employees.
Hybrid Case Manager – Youth & Families Responsibilities
- Conduct comprehensive assessments of members’ behavioral health, social, and developmental needs.
- Develop, implement, and monitor individualized care plans in collaboration with members, families, and Child and Family Teams.
- Identify high-risk factors, gaps in care, and appropriate referrals to clinical case management or crisis intervention.
- Collaborate with supervisors, case managers, Medical Directors, and other professionals to ensure optimal member outcomes.
- Serve as a single point of contact for members to address immediate needs and coordinate services.
- Utilize motivational interviewing, coaching, and education to support members in making informed healthcare and lifestyle decisions.
- Document all member interactions, care plan updates, and outcomes in compliance with regulatory, accreditation, and company standards.
- Participate in case conferences and multidisciplinary meetings to review and enhance member care.
- Maintain knowledge of community resources, Medicaid delivery systems, and Ohio behavioral health services.
Hybrid Case Manager – Youth & Families Requirements
- Bachelor’s degree
- 2+ years experience in children’s mental health, child welfare, developmental disabilities, juvenile justice, or public sector human services/behavioral health.
